Commissioner of Affidavits

A Commissioner for taking Affidavits is an individual who, by virtue of their office or by appointment through the Lieutenant Governor of the Province of Ontario, has the authority to administer an oath and take affidavits, declarations and affirmations.  A Commissioner's stamp is affixed to the document once the oath has been taken and signatures have been sworn.

Those requiring the service of a Commissioner are encouraged to contact the Clerk's Department prior to attending City Hall to ensure a commissioner will be available.  The person swearing or affirming to a document must appear before the Commissioner of Oaths.  Proper photo identification such as a valid driver's licence, current passport or any other government issued photo identification must be presented.  In the event you do not have photo identification, the City Clerk's Office will not commission the document.

The City cannot notarize documents.  A list of Notary Publics and Lawyers who notarize documents can be found in the telephone directory.

Effective January 1, 2016, fees for this service are as follows:

Resident:

$20.00 for first document
$5.00 for each additional document
$40.00 for preparation of an affidavit or declaration

No fee for pension form

Non Resident:

$30.00 for first document
$5.00 for each additional document
$50.00 for preparation of an affidavit or declaration
No fee for pension form
